are created which promote positive change for persons
unable to live on their own.

Using person-centered supports, the homes provide
warm and safe communities for persons needing
a more structured living arrangement.

Case management:

A comprehensive array of care management supports provided to persons transitioning to more independent living include:

• Home visitations & ADL support,
• Assistance maintaining social networks & vocational supports,
• Crisis intervention coordination
• Medication support services.

Family Foster Care

Individualized resources are developed to meet personal
residential needs including companionable and cooperative staff & Friend living arrangements. Care is designed with regard to specific needs and care requirements.
